COVID-19 SITE SIGN IN

This site sign-in has been developed to observe safety practices for COVID-19 in accordance with government requirements.

The sign-in is to be completed by contractors, suppliers and any other persons entering the site.

Every person must complete an individual sign-in every day.

QUESTION 1: Are you required to be in isolation today due to being notified you have been in contact with someone that has returned a positive COVID-19 test in the last 7 days?(Required)
If NO, please proceed to the next question. If YES, please do not enter the site, report to the site supervisor that you cannot undertake the work and return to your place of residence.
QUESTION 2: Do you feel well today and have no cold or flu symptoms or other symptoms of COVID-19?(Required)
If YES, please proceed to the next question. If NO, please do not enter the site. Report to the site supervisor and advise that you cannot undertake the work today and please go and get a COVID-19 test with a negative result before returning to this site.
QUESTION 3: Where there is a requirement in your state or local area to meet Government vaccination criteria for construction workers- Do you meet one of the government's COVID-19 vaccination crieria and have evidence to confirm this?(Required)
If N/A, proceed to the next question. If YES, proceed to the next question. If NO, please do not enter this work site. Contact the site supervisor and advise that you cannot undertake the work until you meet the required Government COVID-19 vaccination criteria,
